Currently no separate insurance required for Ecuador as they abolished separate SOAT policies early this year. Locals now pay it as part of their annual registration fees, while supposedly foreigners entering the country with a valid 90 day TVIP are automatically covered. Insurance for Colombia is readily available at the border with Ecuador on the PanAmerican highway.

If you come into Peru at Copocabana, you will not be able to get insurance until Puno and there are stories of travellers having to catch the bus to Puno and return before being allowed to drive into Peru. More credible are stories of police knowing most foreigners traveling on this road have no insurance, so stopping them and 'fining' them. Same problem coming in at La Balsa crossing from Ecuador.
