I think you have covered all possibilities. We never did get an official receipt so in the end I refused to pay foreigners price because it was obvious the extra was just going into the staff's pockets.
Even when I was happy (on ideological grounds) to pay the foreigners price, we once visited 6 or 7 stations before getting fuel - and that was at the local's price without asking.
Yes, don't let you r tank get too low and carry a jerry can so you can park outside out of camera view.
